Because spongebob is porous, he's always getting things trapped in his body. with patrick the starfish at his side, those two are always jamming things up their noses. take spongebob to the doctor and make sure that his nose is clean and pristine for when he heads over to the krusty krab. the last thing that the customers want on their krabby patty is a big old pile of spongey sea mucus. unless, of course, that's the special ingredient.
